Responsibilities

Patient Care Specialist 1 is a cross-functional role that contributes to AHF's overall mission. The Patient Care Specialist 1 position is intended for someone who has a team attitude and is willing to lend a helping hand when needed. The cross-training and cross-functionality of this position enhance the experience of fellow employees and contributes to an overall team environment. Within this role, individuals will be cross-training in front office duties, patient benefits and patient referral processing.


Front Office duties involve excellent attention to detail when pulling up patient charts and medical records and disclosing benefits and referral information. This level of detail is critical in helping retain patients in care and helps provide an excellent patient experience.


Benefits duties involve presenting a friendly and helpful face to patients that need assistance in completing applications and obtaining pertinent information from family members and/or significant others as it relates to benefits and financial information. Lending a helpful hand at other Healthcare Centers as needed and always being collaborative in any environment.

 

Referral duties involve processing specialty referrals in the most timely and accurate manner to optimize care and to address clinical needs; providing all pertinent information to patients for timely appointment scheduling; assisting with appointment scheduling as patients require; and obtaining specialist consults and scanning into the patent’s EMR to provide the most complete picture of a patient’s clinical condition. They will track and monitor the status and progress of the referral being completed. They will notify a provider of a patient’s lapse in completing the referral and will remain in contact with the patient to improve adherence.

 

The Patient Care Specialist adds consistent value to the Healthcare Center Team by adapting to various situations with ease and by being flexible.
