POSITION SUMMARY: The overall purpose of the Medical Records Clerk position is to establish and maintain organized electronic medical records and files, according to acceptable policies and procedures of the agency. The Medical Records Clerk supports the HCC by ensuring that patient information is properly collected and uploaded into the electronic medical records in a timely and accurate manner.



  • On a daily basis, collect patient documents from the front desk registrar and other team members for uploading into the patients' electronic medical records. Documents include Ryan White eligibility documents, enrollment paperwork, acuity screens, service plans and other documents as assigned.
  • Collect, maintain, upload and organize relevant documents for timely and consistent data entry into the electronic chart and other medical data systems.
  • Assist PCS1 and patient benefits specialist with benefits coordination data entry as needed.
  • Assist with front desk coverage in the absence of the front desk registrar.
  • Attend all trainings for position role as well as for front desk and patient benefits positions, and become knowledgeable and proficient in performing the duties of these other roles when necessary and assigned.
  • Develop effective working relations and cooperate with the medical and support services team.
  • Work closely with the office administrator, quality coordinator, medical and support service teams to ensure that patient data is being collected, entered, and maintained in accordance with grants and other requirements.
  • Participate in quality control chart audits.
  • Participate in the quality management committee, and other pertinent staff meetings, huddles, etc.



This position does not have any supervisory responsibilities



Possess a minimum of High School Diploma and/or 2 years of data entry and data collection experience



  • Basic 10-key data entry skills
  • Basic typing skills
  • Inclination to work independently for prolonged periods of time
  • Superior time management, prioritization, and multi-tasking skills.
  • Proficiency in database systems, electronic medical record systems, etc.
  • Ability to obtain CPS database user proficiency
  • Ability to work independently and as part of a team
  • Flexible
  • Detail oriented with keen ability to spot errors.
  • Highly organized and able to manage multiple tasks.
  • Understands and maintains confidentiality of patient health information.
  • Strives for continuous quality improvement and integrity of data entry processes
  • Computer proficiency in Windows operating systems, Microsoft Office (Word, Excel, PowerPoint, Outlook, etc.), email, Internet, and client databases
